सि.नं नाम थर स्थायी ठेगाना राहदानी नं र बहाल रहने मिति नेपालमा कार्यरत संस्थाको नाम ठेगाना र फोन नं इजाजतप्राप्त व्यक्तिको परिवार वा नजिकको नातेदारको सम्पर्क व्यक्तिको  नाम र ठेगाना, कैफियत
             
             
             

नोटः भारतीय नागरिकको हकमा राहदानी नभएमा परिचय खुल्ने नम्बर अङ्कित कुनै कागजातमा उल्लेख भएको नम्बर।
